Desert Wise offers out-patient psychotherapy to adult individuals, and we offer both in-person and virtual options.
Note: We are not currently accepting couples (i.e., for couple’s or marriage counseling), and we do not see children/minors.
Our specialties are in treating:
- Trauma [including PTSD, C-PTSD (complex PTSD), MST (military sexual trauma)]
- Emotional, relational, & behavioral aspects of Traumatic Brain Injury (TBI)
- Grief & complicated bereavement
- Depression
- High stakes occupations [e.g., military, veterans, individuals with high level clearances (e.g., TS, SCI, Sigma, YW), medical professionals, therapists, aviators, first responders, business owners, executives]
- Difficulties transitioning to the civilian environment

Payment and Fees
We currently accept the following insurances:
- TRICARE
- TriWest (VA Community Care Network)
- Blue Cross Blue Shield (all commercial lines)
- Presbyterian
- Western Sky Community Care
- United Behavioral Health (Optum)
- Medicare – Part B
We also accept direct/private pay patients. The fee structure varies based on the clinician, with rates ranging from $100-200 per hour. For private pay patients, please view the Good Faith Estimate notice by clicking the image below:
New Patient Paperwork

For in-person patients: Note for new in-person patients: Our providers are located in both suites 1 & 3. Our administrative staff &/or your provider can tell you which suite to go to for your sessions.
New patients need to complete and print the relevant forms below in advance and arrive at your first appointment at its scheduled start time. (This form is fillable, which allows you to type and save your answers.)
If you are unable to complete and print these forms ahead of time, you will need to arrive to your first appointment 45 minutes early to complete the paper packet located in the lobby.
We recognize that the initial paperwork can be time intensive, but we assure you that this is a one-time requirement in service of full disclosure and policy comprehensiveness.

Intake paperwork for new patients.
For virtual patients: New patients need to complete the relevant forms below in advance and send them to their provider at least 24 hours prior to their scheduled intake session. If you do not send your completed paperwork in advance, your intake appointment may be canceled and rescheduled for a later date.
We recommend that patients password protect their paperwork prior to emailing it. If this is not an option, you may exclude sensitive, identifying information to relay to your provider verbally. Alternatively, you may fax your paperwork (i.e., by requesting the fax number from your provider), you may mail it to us at: Desert Wise, 1201 Eubank Blvd NE, Ste. 1, Albuquerque, NM 87112.

One-Time Evaluations
We are experienced in a variety of occupational and diagnostic evaluations, especially those that pertain to:
- Fitness for duty
- Occupational functioning
- Occupational suitability/fitness/retainability
- Disciplinary actions from professional boards
- Assessment of suitability for special positions
- Second opinions
- Diagnostic evaluations
- Assessment of prognosis
Insurance company’s willingness to cover these evaluations varies from one company to the next, as well as one situation to the next. Individuals seeking one-time evaluations are responsible for checking with their insurance company and understanding their out-of-pocket responsibilities.
The fee structure for direct/private pay one-time evaluations varies by clinician. The ranges are listed below:
- $250-500 per evaluation (includes: 45 – 90 minutes face-to-face time, record review, and report writing — up to 6 hours of total work)
- $50-100 per hour after the first 6 hours of total work
Individuals being seen for one-time evaluations need to print and complete the relevant forms below in advance and arrive at your evaluation appointment at its scheduled start time. Additionally, you will need to bring any relevant mental health records to your evaluation.
